1. This offer is limited to dental services provided by the Dental Care and Implant Centre. The voucher can be claimed between the 1st December 2017 and 31st of December.

2. To claim the voucher the recipient has to complete all required fields on the form published on our website www.dentistinkrakow.co.uk.  By providing all required information the recipient confirms that they have read, understood and agree to the terms and conditions

3. This voucher entitles the recipient to receive a free dental consultation package which includes the following: dental examination, treatment plan, OPG x-ray and scale & polish and to receive a 5% discount for all the procedures proposed in the treatment plan prepared by the Dental Care and Implant Centre.

4. The voucher is valid only for the treatments provided by the Dental Care and Implant Centre ul. Balicka branch and for the period ending on the 01st of April 2018.

5. The voucher is single use and issued on an individual bases. This offer is non transferrable.

6. The free dental consultation package included in this special offer can be received only under the condition that the recipient of this voucher will continue the treatment proposed by the clinic’s dentist no later than on the next dayfollowing the consultation.

7. The recipient of this voucher is under no obligation and after dentist consultation, entitled to undergo only the treatment they consider necessary.

8. The recipient reserves the right not to undergo any proposed treatment however in such cases it will incur a charge of 109 GBP to cover for the services included in the dental consultation package.

9. The recipient must book an appointment at least 5 days in advance. The bookings can be made via email (contact@dentistinkrakow.co.uk) starting on the day the voucher was issued but no later than on the 01st of April 2018.

10. Offer expires on the 01st of April 2018.
